Print approval form
A ready-made form for putting customer approval on the record before a job goes to press. A4, one page, to be filled in and signed. Print your own logo on it and use it as your shop’s form — we ask for nothing in return, and you do not need to register.

The six things an approval form needs to be worth anything
Every one of these fields is the answer to an argument that actually happened. Each field left blank is a gap for "that is not how we understood it" to walk into later.
Size and tolerance
"10×15" on its own is not enough. Without the trim allowance and an acceptable tolerance written down, even a millimetre becomes an argument.
Quantity and overs/unders tolerance
Waste happens in print. Writing "15,000 units" without a tolerance hands the bill for a short delivery to the print shop.
Colour and Pantone
Colour on screen comes out differently in print. The Pantone number and a "sample required for colour accuracy" warning belong on the form.
Material and unwind direction
Unwind direction on labels, weight and stock type on paper. A roll that comes out wound the wrong way cannot be loaded and is a total write-off.
Die and finishing
Die number, varnish, foil, cutting. Without the die number written down, nobody remembers which die was used when the repeat run comes.
The identity of the file
The most commonly skipped field. Without the file name, the version number and the date, the form says "some artwork was approved" but never says which.
One signature does not cover everything
Most approval forms have a single signature at the bottom, and what that signature covers is anyone’s guess. This is where "I approved it, but the quantity was wrong" comes from: the customer looked at the artwork and signed, and never read the quantity at all.
On this form six items are ticked separately — artwork, size, colour, material, quantity, deadline. Each has three options: I approve, I need a correction, I want a sample. A row left unticked counts as not approved.
How far does a paper form get you?
We made the form and it genuinely works. But let us name its limits too — a limit you do not know about shows up on the day you need it not to.
BaskıGATE exists to close exactly those three gaps: the same six items are approved through a single link the customer opens on their phone; the approval is bound to the fingerprint of the file and the details; and if either one changes, it falls away by itself.
Questions about the form
Can I use the form under my own shop’s name?
Yes. The dashed frame at the top left is there for your logo or stamp. You are free to reproduce it, change it and hand it to your customers. You do not need to credit us or ask permission.
Is there a Word or Excel version?
PDF only for now. If you want to change the field layout, write to us and we will prepare an editable version.
Is this form legally binding?
A signed document is a record of the agreement between the parties and can be submitted as evidence in a dispute. That said, we are not lawyers and this form is no substitute for legal advice; for a job that matters to your contracts, show it to yours.
Can I change the warning text?
You can, but we would keep three points: what the signature means, that differences raised after signing count as new work, and that screen colour can differ from print colour. All three answer the arguments that come up most often.
Do I have to give you my email address?
No. The file downloads when you press the button; there is no form to fill in, nothing to register for and nothing to accept.
